The PrimeSource VisionCare Plan offers seniors the convenience and comfort of outstanding ocular healthcare in the comfort of the their care facility. The PrimeSource Optometry Care Team has the extensive experience in geriatric eye care to provide the best possible vision services quickly and efficiently. A comprehensive, on-site eye examination is the cornerstone of our program to maximize functional vision and maintain the resident’s independence. PrimeSource eye examinations may include:

  • Visual acuity (How well a person sees at near and far ranges).

  • Visual field (Peripheral vision).

  • Examination of the eye’s surface and interior structures.

  • Measure interior eye pressure (Intraocular pressure).

  • Refraction (Helps determine what kind of visual correction might be needed).

A PrimeSource-affiliated Optician will dispense and fit eyeglasses and provide an eyeglass storage case to the resident. Optometry services not covered by Medicaid are available on a fee-for-service basis, with or without the liability program.

  • Red, watery or dry eyes, eye discharge, visual hallucinations.
  • Frequent stumbling or falling.
  • Rubbing eyes, abandoning reading or writing.
  • Decrease in socialization.

  • Lenses are ground for each patient’s unique prescription.

  • Patient can choose from up to 100 metal or plastic frame styles.

  • Patient receives a vinyl storage case with their glasses.

  • Patient’s name is engraved on eyeglass frame.

  • PrimeSource Technician will adjust frames on-site for better fit and more comfort.

TotalCare™ Replacement Guarantee for Eyeglasses
In the event a patient’s eyeglasses are lost or broken beyond repair within two years of the date they were first delivered to the patient, regardless of who manufactured or delivered the eyeglasses to the patient, PrimeSource will provide unlimited replacement at no cost to the patient, the patient’s family or the facility.*

Warranty for Eyeglasses

If eyeglasses manufactured by, or for, PrimeSource Healthcare are found defective, please notify PrimeSource within 90 days of the delivery of the item. PrimeSource will repair or replace the eyeglasses without charge to the patient, the patient’s family or the facility

30-Day, Money Back Guarantee
PrimeSource Healthcare will issue a refund for eyeglasses returned for any reason within 30 days from the delivery date. A full refund (100%) is issued for eyeglasses returned in “like new condition”, and with all accessories. A 10% restocking fee is assessed for eyeglasses not returned in “like-new” condition, or without all the accessories.

Early Exams May Prevent Vision Loss

Who has a special need for periodic eye examinations? Seniors. Why? Approximately one person in three has some form of vision-reducing eye disease by the age of 65. With early warning of eye disease and proper treatment by the PrimeSource Optometry Care Team, seniors could be spared visual handicaps which threaten their independence and limit their enjoyment of life.

The most common causes of vision loss among the elderly are:

  • Age-related macular degeneration - A common eye disease that gradually destroys sharp, central vision and may lead to a loss of vision in one or both eyes.

  • Cataract - A clouding of the eye’s lens that causes loss of vision.

  • Diabetic retinopathy - A complication of diabetes and a leading cause of blindness. It occurs when diabetes damages the tiny blood vessels inside the retina, the light- sensitive tissue at the back of the eye.

  • Glaucoma - An eye disease in which the normal fluid pressure inside the eyes slowly rises, leading to vision loss or even blindness.

Risk increases with age. Regular eye examinations are critical. Cataracts need to be monitored and eventually removed. Glaucoma, macular degeneration and diabetic retinopathy all sneak up on their victims, destroying eye cells before the person is aware there’s anything wrong. No ifs, ands, or buts, seniors should be examined by the PrimeSource Optometry Care Team on a regular basis.

*To be eligible for the TotalCare™ Replacement Guarantee, a person must reside in a facility contracted with PrimeSource for all four services, consent to all four PrimeSource services and have had initial visits with each PrimeSource-affiliated Provider. TotalCare Guarantee benefits are terminated if a person no longer resides in a contracted facility. PrimeSource Healthcare may seek payment from Medicaid, other insurance or liability when appropriate.

**Eyeglasses for Ohio patients may be produced in an external optical laboratory.
