Who is Responsible for Resident Consent?


PrimeSource Healthcare prefers to partner with the facility for the resident consent process. We request the facility provide documentation about all residents, which includes:

  • Current census.

  • Copies of all face sheets.

  • Completed profile and demographic sheets.

  • Sufficient supply of facility letterhead and envelopes for a mailing to each resident or responsible party.

PrimeSource Healthcare will mail consent notification letters to residents or responsible parties in order to introduce our healthcare company, provide information about the TotalCare™ Guarantee, and inform the reader of the right to choose a different provider of services.

Please note: Each resident is responsible for notifying the facility of his/her intent to use another provider. This notification must be forwarded by the facility to PrimeSource so that we can maintain an accurate database for your facility.

When a facility contracts with PrimeSource Healthcare, we become the provider of choice. All residents are deemed to have consented to all our services, unless a patient indicates a desire to choose another provider. This procedure is consistent with other ancillary service providers’ forms of consent, such as pharmacy and laboratory.

For new admits, PrimeSource Healthcare will provide brochures and consent notification letters to place in the facility’s admission packet. If the new resident selects a different provider, please forward the signed consent notification letter to PrimeSource.

PrimeSource Healthcare has sophisticated software packages that schedule, report and manage our patients’ care. Our staff is able to track all residents in an expanding multi-service environment. We can provide a multitude of documentation for a facility's records, including:

  • Schedule report.

  • Established resident report.

Does PrimeSource Healthcare Credential?


YES.
The PrimeSource-affiliated Provider is an integral part of the healthcare team within the subacute and long term care facility and accordingly, must abide by governing rules and regulations that optimize resident care. PrimeSource conducts peer review of the training, experience, malpractice coverage and similar relevant information to approve a Provider who wishes to practice through PrimeSource Healthcare.

Does PrimeSource Healthcare Perform Utilization Reviews?


YES.
A random sampling of each provider’s examinations are reviewed quarterly for appropriateness of healthcare services, on a prospective, concurrent or retrospective basis.The PrimeSource network of healthcare professionals is comprised of ethical specialists dedicated to providing medical services to residents of long-term care facilities. PrimeSource-affiliated Providers offer treatments designed to minimize problems and maximize resident independence.

Is PrimeSource Healthcare Audit Sensitive?


YES.
Federal and state legislation has set forth requirements for subacute and long-term care facilities to participate in the Medicare and Medicaid Programs. In Regards to HIPAA Regulations: Are PrimeSource Associates Allowed Access to Patient Health Information?

YES. As healthcare professionals, we understand that health information is sensitive and personal. While everyone at PrimeSource Healthcare is committed to the privacy of patients, in order to serve them, we need to gather, keep and use the health information typically documented on face sheets or patient registration forms.

PrimeSource's healthcare professionals and Associates are trained in the privacy regulations of the Health Insurance Portability and Accountability Act (HIPAA) and aware of the rules and limits on who can look at and receive patient information. HIPAA does, however, have provisions to allow access to information when used for healthcare purposes by providers such as PrimeSource Healthcare.

PrimeSource wants you to know the health information we use is for providing treatment and coordinating care of our patients. In addition, PrimeSource may use the information to obtain/seek payment for our services. Be assured PrimeSource will not use or disclose more health information than is absolutely necessary, and only to those who need access to it. What is the Liability Program?

Under Medicaid Policy (Manual Section 3455.15.10 “Deductions From Income in Post Eligibility”), when a Medicaid recipient requires or has a medical need not covered by Medicaid, then in such event, the resident’s income may be applied to cover the cost of such medical need, (i.e., dentures, hearing aids or eyeglasses). While these prosthetics/medical devices are generally covered, there are times when Medicaid may deny coverage. In these cases, when medically necessary, PrimeSource will pursue payment through the Liability Program.


In cases when a resident’s income is offset to pay for dentures or other non-covered medically necessary care, Medicaid makes up the difference leaving the facility fully reimbursed.

When a PrimeSource-affiliated Provider examines a resident whose treatment plan falls under the Liability Guidelines for Reimbursement, PrimeSource will obtain consent from the resident or responsible party. Upon receiving consent, PrimeSource will provide the treatment or manufacture the prosthetic or medical device.

PrimeSource forwards all required documentation to the facility and to the resident’s caseworker. The caseworker approves the charge and initiates the diversion of the resident’s income (“liability”) to pay for the service or prosthetic. This is termed“reducing the resident’s liability to zero”.

Liability requests for insurance co-payments or balances will be based on the general consent for services.

The Certified Dental Assistant is responsible for assisting PrimeSource dentists in the delivery of dentistry services including:

Medical Biller – Deerfield IL

Description

As a member of our team, we will depend on your outstanding skills to:
1. Proper billing of clinical encounters.
2. Determining financial eligibility of patients.
3. Obtaining necessary authorizations when billing private pay patients and insurance carriers as established by carrier and/or PrimeSource policy.

Description

The Clinical Assistant is responsible for several important patient care tasks and plays a critical front line role that requires great communication, customer service and organizational skills.
