Customer Service Representative - Patient Care Representative

Responsibilities

We are currently looking for an out-going and enthusiastic Patient Care Representative who possesses tremendous people skills and can work well in a fast-paced environment.  The ideal candidate will have great decision making skills, the ability to work independently of direct supervision, and first rate organizational skills that allow them to manage multiple tasks and see them all through to completion.  Duties include:

  • Schedule clinical visits with designated contacts at our contracted facilities.
  • Facilitate the completion of required paperwork prior to physician visits to the facility.
  • Coordinate visit schedules with all field associates and physicians.
  • Confirm patient eligibility for scheduled visits.
  • Respond to and coordinate urgent care for patients.
  • Communicate with facilities contacts to confirm times of visits and that patients are properly prepared.
  • Respond to all incoming communications from facilities, patient families, patients, and field staff as appropriate.

  • Able to pass drug screen, employment and criminal background check.
  • The ideal candidate must possess the ability to interact effectively in varied nursing facilities with varied PrimeSource team members on a daily basis.
  • In addition, daily travel is required in a company car. Candidate must have the ability to work in locations up to 2.5 hours away from home and complete 12 hour work days (including travel) at least 50% of the time.
