About PrimeSource Healthcare

A lot has changed since we launched our healthcare company more than 20 years ago. But the one thing that will never waiver is our commitment to bringing the best personalized and compassionate care to our patients while treating them like family. We call it our “patient first” philosophy.

PrimeSource Healthcare's mission is to provide treatment and services that maximize the well being of the residents of the long-term care and retirement facilities. Our care meets or exceeds professional standards and is grounded in the sound clinical judgment of our affiliated practitioners.

At the heart of PrimeSource Healthcare are experienced and credentialed Audiologists, Dentists, Optometrists, Podiatrists; and highly-trained Technicians and Patient Care Associates. They personify the patient-focused care for which we are known. All are dedicated to our mission of providing healthcare that helps our patients minimize the effects of aging and enjoy a better quality of life.

Clinical Conduct
At PrimeSource Healthcare, we acknowledge and understand the basic premise of which services Medicare and Medicaid will provide compensation. PrimeSource-affiliated Providers will perform only medically necessary, medically justified, and medically reasonable services.

TotalCare™ Replacement Guarantee

Life can be unpredictable: Eyeglasses can be crushed under a wheelchair, a hearing aid can be misplaced, or dentures can be damaged after falling from the bed stand. All are mishaps that could cost thousands of dollars, but not for many of the patients of PrimeSource Healthcare. In many instances, our eyeglasses, dentures, hearing aids and diabetic shoes* are covered by the PrimeSource TotalCare Replacement Guarantee. It provides unlimited replacement of lost or broken beyond repair prosthetics. The patient, the patient’s family and the facility pay nothing to replace these medical devices when they are covered by the guarantee.**

The PrimeSource TotalCare Replacement Guarantee is another example of our “patient first” philosophy in action. We eliminate waiting for an insurance company to authorize a new hearing aid or budgeting to purchase dentures. Instead, our patients get a quick, no-hassle replacement. It is important to our care teams that patients always have use of the medical devices needed to improve their quality of life.

*In order for diabetic shoes to be covered by the TotalCare Guarantee, the facility must use PrimeSource as its shoe vendor.

**To be eligible for the TotalCare™ Replacement Guarantee, a person must reside in a facility contracted with PrimeSource for all four services, consent to all four PrimeSource services and have had initial visits with each PrimeSource-affiliated Provider. TotalCare Guarantee benefits are terminated if a person no longer resides in a contracted facility. PrimeSource Healthcare may seek payment from Medicaid, other insurance or liability when appropriate.

PrimeSource Labs
PrimeSource Healthcare maintains its own labs to produce eyeglasses, natural-looking dentures and digital hearing aids that meet strict quality standards. Some service providers offer an off-the-shelf solution. Not PrimeSource. Each prosthetic or medical device is crafted by our own lab technicians to achieve exacting quality standards and customized to meet the unique needs of the patient.

Company History
Dr. Richard Fleming (pictured above) established his private practice in 1946, to which PrimeSource Healthcare is the follow-on successor, with roots in providing mobile, on-site vision care to seniors at long-term care facilities. Based upon principles of uncompromising care, the Company flourished and continued to grow for decades.

In 1994, David Fleming acquired the Company. He transformed PrimeSource from an organization with a handful of employees, operating in a limited number of facilities to the nation’s leading provider of mobile, on-site geriatric services.

Today, the Company serves well over 37,500 patients at nearly 400 contracted long-term care facilities in Indiana and Ohio.
